During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor. We accept most insurances.
After making an appointment, we will provide you access to your Patient Portal where you will be able to view and update all your medical records.
*Minors must be accompanied by their parent or guardian at their New Patient Appointment at Moore Foot & Ankle Specialists, PA. At this time, the minor's parent or guardian will be able to complete an Authorization to Consent to Health Care for Minor Form for future appointments for someone else to bring the minor patient to their appointments.
